If you are a broker or agent interested in the Assisted Living Federation of America Affinity Insurance Program, brokered and administered by Marsh Program & Franchise, please complete the following steps to secure Marsh sub-producer status:

Step 3:
Please provide current copies of:

• The agency license and principal owner/officer license documentation for all states in which business with Marsh is or may be conducted. Agency principal's title must also be included.
  
  
• A Certificate of Insurance showing the policy number, policy term, and liability limits of at least $1 million per claim and $1 million aggregate.
  
  
• A completed W-9 form including tax ID number.
