DISABILITY INCOME PLAN


When a covered disability accident or illness such as a heart condition, maternity complications, surgery or chronic back pain prevents you from working, this plan provides an income to help see you through until you're ready to go back to work. To best suit your needs, you choose from 3 monthly benefit amounts. Underwritten by the Hartford Insurance Company.

Disability Claim Examples

A 55 year old male member was disabled as a result of an automobile accident. This accident resulted in head injuries, concussion, headaches, back and neck complaints.

A 52 year old female member has been disabled due to a torn ligament in her knee as the result of an on-the-job accident. Her activities are very limited in that she cannot climb stairs or walk more than 2 blocks. She requires the use of a knee brace and a cane.

A 50 year old member has received disability benefits since January 1997. She is disabled as a result of migraine headaches, myalgia, and a jaw disorder. She is unable to work since the jaw problem, which makes speech difficult. The myalgia makes movement painful and the migraine headaches are severe.
